Why Complete Forms Ahead of Time
Filling out your paperwork before you arrive saves you time in the waiting room and gives our team a head start on reviewing your health history. Dr. Williams can spend more of your appointment focused on your exam and any questions you have, rather than waiting for intake paperwork to be processed.
If you prefer to complete your forms in person, simply arrive 10 to 15 minutes before your scheduled appointment time. Our front desk team will have everything ready for you, and our bilingual staff can assist patients who prefer to communicate in Spanish.
What to Bring to Your Visit
Even if you complete your forms online, please bring the following to your appointment:
- A valid photo ID
- Your insurance card (if applicable)
- A list of current medications
- Any recent dental X-rays from a previous provider

After checking in, you will meet with Dr. Williams for a comprehensive exam that includes digital X-rays, a professional cleaning, and an oral cancer screening. We take time to discuss your dental history, any concerns you have, and your goals for your smile. If treatment is needed, we will explain every option, walk you through the expected timeline, and provide a clear cost estimate before moving forward.
Our office accepts most PPO dental insurance plans, and we verify your benefits before your visit so there are no billing surprises. Patients without insurance can ask about our in-house membership plan or flexible financing through CareCredit, Cherry, Affirm, and Klarna.
